Dr. Walrath participates in nearly every major insurance plan. If you have an insurance and Dr. Walrath is out-of-network, his office will help you to file for out-of-network benefits.

Dr. Walrath is pleased to see patients in Atlanta (Sandy Springs), Cumming, Lawrenceville, and Gainesville. He performs minor office procedures in all of those locations. Major surgical procedures are performed in Atlanta, Alpharetta, Cumming, Gainesville, and Covington.

You can schedule to see Dr. Walrath in his offices at The Woolfson Eye Institute, with most of his time at WEI spent in the Atlanta / Sandy Springs location.
Dr. Walrath also practices in affiliation with Gainesville Eye Associates (Gainesville).

Surgical Forms
Below are some forms that are helpful in preparation for surgery:
- Certain medications need to be stopped prior to surgery because they increase the risk of bleeding-related complications. Excessive bleeding can lead to a compromised outcome, may lead to a higher reoperation rate, and in exceedingly rare instances can lead to visual loss.
- Medical clearance form: a form for your primary medical doctor to review and complete to ensure that your surgery can be done safely, in the context of your overall medical condition.

Insurance Plans
Dr. Walrath participates in nearly every major insurance plan. If you have an insurance and Dr. Walrath is out-of-network, his office will help you to file for out-of-network benefits. Before a procedure is scheduled, your insurance coverage will be verified. Please feel free to send me an email, understanding the following:
- Do not send me an email if you have an urgent problem. Urgent problems need to be handled over the phone. If you are a postoperative patient of mine with an urgent problem, please call the office immediately. If your problem is an emergency unrelated to the surgical region, please call 911.
- Email cannot substitute for a face-to-face encounter with patients. There are limits to the advice that I can dispense over email, particularly to individuals whom I have not met face-to-face.
- Email in general may not be answered promptly.
- Email is not a secure means for transmission of sensitive information, and by choosing to email Dr. Walrath, you recognize this and relinquish any protections regarding sensitive information that you choose to send.

FAQS
Pure cosmetic consultation costs $100. This fee is discounted from your office or surgical procedure should you elect to proceed with a treatment. Consultation to evaluate true eyelid malposition and to determine if the patient meets insurance requirements requires clinical evaluation and occasionally special testing — these consultations are billed to insurance at the standard rate.

Our oculoplastic doctors will be able to tell you if they believe your condition meets the criteria for insurance reimbursement. In the case of upper eyelid surgery, each insurance carrier has slightly different criteria. These criteria are published and available, and based on your examination and insurance, Our oculoplastic doctors can usually predict when the insurance company will pay for your upper lid surgery. In plans that preauthorize, you will have the insurance company decision before your surgery, so that there are no surprises.
